Product Description: A comprehensive surveillance plan for pandemic influenza that incorporates two components: 1) clinical case-based surveillance and 2) laboratory-based surveillance.

Target: Hospital-based Healthcare Workers

Initial Award Period: (March 9, 2009 – September 8, 2010)
Activities included: creating a Severe Influenza Pneumonia Multidisciplinary Multi-hospital Task Force; developing a surveillance data collection form and instruction manual; and developing a web-based program with the capability to enter surveillance data via the internet.

Final Award Period: (September 9, 2010 – March 9, 2012)
Activities will be centered on the following: continuing surveillance via web-based reporting and lab testing; updating the project website to include more near-real-time reporting mechanisms; surveying infection preventionists networks to improve communication and information dissemination; and hosting a state-wide presentation highlighting the results of this project and future plans.
